Description Mountain View Hospital is looking for a Social Work Case Manager to join our team!
JOB SUMMARY: This position provides support for the LCSW. And requires a passionate commitment of core values and behaviors. Responsibilities- Coordinates and assists with new patient set up and resource contact for LCSW. Functions as a Case Manager for the Social Worker. Duties And Responsibilities Title: SOCIAL WORK CASE MANAGER - Helping the LCSW in assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plan.
- Facilitates flow of information regarding patient’s course among care providers.
- Keeps strict confidentiality with patients information and follows all HIPAA laws
- Listening to clients’ concerns and providing resources or referrals as required.
- Following up with patient’s progress periodically, collaborating with the LCSW and making adjustments as needed to improve outcomes.
- Keeps a spreadsheet of patients who have been referred out for counseling.
- Helps schedule patients to be seen with the LCSW for counseling.
- Scans/Copies and uploads documents into EMR Aria and Cerner.
- Helps medical staff provider resources to patients.
- Maintains client contact list. And updates folders as needed.

Please note benefits are based on eligibility according to full-time, part-time or PRN status classification.
Qualifications Education/Certifications: Must be a high school graduate or an accredited secondary educational system. Preferred C-SWCM (certified social work case manager) must have a working knowledge of the Microsoft Office suite products (Word, Excel, & Outlook). Knowledge of basic Workers Compensation and medical terminology is essential. Must have a current driver’s license.
Experience: Knowledge of Medical practice, and Medical Terminology.
