Job Description

Title - Quality Assurance Nurse (RN/LPN)

Location - Flagstaff, AZ

Position: 1 Full-Time

Schedule: Flexible; options include 7:00 AM – 3:00 PM, 8:00 AM – 4:00 PM, or 8:00 AM – 5:00 PM, Monday – Friday. Some weekends may be required.

Overview:
We are seeking a Licensed RN or LPN to serve as a Quality Assurance Nurse , overseeing staff education and infection prevention. This role ensures compliance with regulations, evaluates care quality, and provides education to maintain high standards of resident care.

Key Responsibilities:
  • Implement and monitor policies and procedures for quality assurance using regulations, audits, and evidence-based practices.

  • Analyze data from audits, interviews, and reports to ensure compliance across all disciplines and departments.

  • Review monthly CASPER reports and develop QAPI-based Performance Improvement Plans (PIPs) as needed.

  • Prepare and interpret reports, statistical analyses, and data summaries for staff.

  • Assist managers in corrective action plans and monitor deficiency corrections.

  • Ensure compliance with state, federal, and VA regulations .

  • Investigate incidents of non-compliance, abuse, neglect, or mistreatment of residents.

  • Coordinate facility surveys and internal committee meetings, including Quality Assurance and Infection Control Committees .

  • Identify staff training needs and implement educational programs for quality and compliance.

  • Conduct infection control surveillance and track infections using McGeer and non-McGeer criteria .

  • Stay current with CMS, CDC, and other regulatory guidelines.

Knowledge & Skills:
  • Management and administrative principles

  • Facility programs, policies, and procedures

  • Quality assurance and utilization review methods

  • Nursing home regulations (OBRA and other applicable standards)

  • Basic statistics, data gathering, and staff training techniques

  • Adult education methods and competency assessment

Skills:

  • Verbal and written communication

  • Problem-solving and analytical thinking

Abilities:

  • Analyze and interpret information and data

  • Work effectively with all levels of staff

  • Develop and coordinate training programs

Requirements:
  • Active RN or LPN license in Arizona

  • DPS Fingerprint Clearance Card

  • Infection Preventionist Certification

  • Valid Arizona Driver’s License for in-state travel
