Job Description

University Home Care Inc is seeking a compassionate and dedicated Medical Social Worker to join our team in Livonia, MI. In this role, you will provide support and advocacy for patients and their families, helping them navigate the complexities of healthcare and social services. You will collaborate with medical professionals to ensure comprehensive care plans that address both the emotional and social needs of our clients. Responsibilities

• Assess patients’ social, emotional, and financial needs related to their medical conditions.
• You will be visiting client in their home.
• Develop and implement care plans that connect patients to community resources and support services.
• Advocate for patients’ rights and help them access necessary healthcare and social services.
• Provide counseling to patients and families coping with diagnoses, treatment, and health-related challenges.
• Work closely with interdisciplinary teams including physicians, nurses, and other healthcare providers.
• Maintain accurate and confidential records of patient interactions and case management activities.
• Participate in discharge planning and coordinate follow-up care to ensure continuity of support.

Requirements

• Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program.
• Licensed or eligible for licensure as a Social Worker in the state of Michigan.
• Experience in medical or healthcare social work preferred.
• Must be able to travel to client home
• Strong interpersonal and communication skills.
• Ability to work collaboratively in a multidisciplinary healthcare environment.
• Compassionate approach to patient care and sensitive issues.
• Excellent organizational and documentation skills.
